Myth: Exercise physiology and physiotherapy are essentially the same thing.
Truth: While there is overlap in the populations we serve, the two professions serve distinctly different clinical purposes.
Physiotherapy typically focuses on acute and sub-acute assessment, manual therapy, soft tissue treatment, and early-stage injury management.
Exercise Physiology focuses on structured therapeutic exercise prescription for people with diagnosed health conditions, with an emphasis on building functional capacity over time, managing chronic conditions, and facilitating return to work and daily function.
In practice, this means EP becomes most valuable in the sub-acute to chronic phase, once manual therapy has achieved its ceiling, and the primary clinical need shifts from reducing acute symptoms to rebuilding capacity.
For referrers, this distinction matters. Ongoing physiotherapy for a patient 12 months post injury who has plateaued functionally is a different clinical question from an EP referral to build progressive capacity for the same patient.

Post-operative rehabilitation is one of the most well-supported clinical indications for hydrotherapy, and one of the most underutilised in private practice.
Following joint replacement surgery, spinal procedures, or ligament reconstruction, the primary challenge in early rehabilitation is applying sufficient load to drive healing and neuromuscular adaptation without exceeding the tolerance of surgical repair.
On land, this window is narrow. In water, it opens significantly.
Buoyancy reduces compressive load on the operated joint while allowing the surrounding musculature to work meaningfully. This enables earlier mobilisation, earlier strength work, and earlier cardiovascular conditioning than would be safe with full body-weight loading.
Specifically, hydrotherapy post-surgery supports:
Earlier restoration of joint range of motion through movement in a reduced-load environment. Reduced post-operative swelling through hydrostatic pressure, which gently assists lymphatic return. Earlier gait training and weight-bearing practice in a protected environment. Cardiovascular conditioning without the impact risk of land-based exercise.

One of the most common conversations we have at Rehab Insight is with people who have been told their burnout is "just stress" and to take some time off.
Rest helps. But rest alone does not resolve the physiological depletion that underlies true burnout.
Sustained stress dysregulates the HPA axis. Cortisol rhythms become disordered. Sleep architecture is disrupted. Inflammatory markers elevate. Muscle mass reduces. Cognitive function deteriorates.
These are measurable biological changes, not character failings.
What the research supports for burnout recovery:
Structured, graded physical activity at an appropriate dose. Not high intensity. Not rest. A carefully prescribed program that rebuilds physiological capacity without overwhelming an already-depleted system.

Did you know that fear-avoidance behaviour, the tendency to avoid movement because of anticipated pain, is one of the strongest independent predictors of long-term work disability in musculoskeletal presentations?
And critically, it is independent of how much pain a person actually has.
A worker with moderate pain and high fear of movement is more likely to experience prolonged disability than a worker with significant pain and low fear-avoidance. This is well-supported in the occupational rehabilitation literature.
For case managers and GPs, this means asking not just "how much pain is this person in?" but "how is this person relating to movement and to the possibility of working?"
The clinical response to fear-avoidance is structured graded exposure under EP supervision: carefully reintroducing feared movements at a manageable level, building repeated evidence for the nervous system that they are safe, and progressively expanding functional capacity from that foundation.

Myth: Hydrotherapy is basically aqua aerobics with a different name, primarily useful for elderly clients with arthritis.
Truth: Clinical hydrotherapy is an individually prescribed rehabilitation program, supervised by an Exercise Physiologist, used across a wide range of presentations and age groups.
Post surgical rehabilitation for joint replacements and ACL reconstructions. Acute low back pain where land-based loading is too aggravating to tolerate. Chronic pain conditions where nervous system sensitisation makes standard land exercise difficult. Neurological conditions affecting balance and coordination. Athletes returning from injury who need to maintain conditioning without impact.
The water is the medium. The clinical reasoning behind what you are doing in it is what makes it rehabilitation rather than recreation.
What changes in the water:
Buoyancy reduces effective body weight by up to 90 percent depending on depth. Joint stress is dramatically reduced. Range of motion improves.
