Sleep and Pain 🌛
😴 Many with persistent pain have difficulty falling or staying asleep most nights of the week leading to changes in their daily functioning.
🌙 Chronic sleep deprivation can have long-term health consequences and is worth working on. Cognitive Behavioural Therapy for Insomnia (CBT-I) should be a first-line treatment. Sleep hygiene alone is often ineffective.
👀 If someone has a sleep issue, here is a potential starting place after they have been cleared of any other medical diagnosis such as narcolepsy, restless leg syndrome or sleep apnea:
🧠 “Overanxious, underslept.” Sleep issues and anxiety often ride together. Perhaps a referral to a psychologist is key to assist in unpacking anxiety.
1️⃣ Restrict the total time spent in bed - many will go to bed earlier or stay in bed longer in the morning in attempt to provide more time to get enough sleep. Set goals (i.e. aim for 6 hours, then 6.5, etc) and embrace the 'Road to Recovery' that is filled with good and not good times.
2️⃣ If you are awake for longer than 20-30 minutes, get up, move to another room, and read a book. Once you feel sleepy, return to the bed.
3️⃣ Sleep environment - make the room cool, dark, and comfortable.
4️⃣ Routine - create conditioned cues for sleep. Do the same relaxing routine every night.
5️⃣ Remove inhibitors - this may be cannabis and alcohol. They help you fall asleep but keep you in the lighter stages of sleep (1 and 2) leading to fragmentation. Others are cellphone use, emails, and other stimulating tasks.
6️⃣ Others - get people moving/exercising more and consider activities that enhance mood (mindfulness, music, reading, exercise, etc).

Following my post on Monday after my 2nd COVID vaccine I had many comments about the risks especially blood clots
You may have heard or read about the Astra Zeneca vaccines being placed on hold due to some concerns about blood clots
However to put this risk into context we need to look at the risk of blood clots with other things
And a recent systematic review demonstrated that the biggest risk of blood clots is actually the COVID virus itself, not the vaccines
Again I can understand the concern and doubts many have when the media and others keep talking about these risks
But try to look at the science and data and consider the risks of NOT having the vaccine just as much as having it!
😎 Disc protrusion completely resolves on its own!
👋🏻 Here's a case report of a 28 year old with severe back and leg pain
📸 MRI November 2018 showed “Large L4/5disc protrusion w severe cord narrowing/ crowding of cauda equina rootlets”....
🕒 7 months later and a new round of imaging
📸 MRI May 2019 showed reabsorption and now pain free! 😱
➡️ This is a great example of conservative Mx & ‘watch & wait’ approach. We don't know exactly what his treatment consisted of, but we know from the research that disc problems resolve on their own
📚 A meta-analysis (Zhong et al 2017) has shown that 2/3 of disc bulges spontaneously «heal» on their own! Many of them within a few months too, not years
