Testosterone After 50: It Matters for Men.
Testosterone is often called the male s*x hormone, but its role goes far beyond libido and s*xual performance. For men above 50, maintaining a healthy testosterone level is important for muscle, bone, energy, metabolic health, mood and healthy ageing.
What does testosterone do?
Testosterone is mainly produced in the te**es, under signals from the brain and pituitary gland. It supports:
Muscle mass and strength, helps maintain and build lean muscle.
Bone strength, supports bone mineral density and may reduce age-related bone loss.
Sexual health, important for libido and contributes to normal s*xual function.
Red blood cell production, stimulates production of RBCs.
Body composition : low testosterone is associated with reduced muscle and increased body fat.
Mood and wellbeing : adequate testosterone supports motivation, energy and general wellbeing.
Metabolic health : testosterone and insulin sensitivity are closely connected. Obesity, particularly excess visceral fat, is commonly associated with lower testosterone.
What happens to testosterone after 50?
Testosterone generally declines gradually with ageing, but age alone does not mean a man must have low testosterone.
Poor sleep, obesity, insulin resistance, chronic illness, excessive alcohol, severe calorie restriction, nutritional deficiencies and certain medicines can further suppress testosterone.
Therefore, a man should not be judged only by his age. Symptoms + laboratory results + overall health need to be considered together.
What can low testosterone look like?
Persistently low testosterone may be associated with:
Low energy → reduced motivation → reduced libido → erectile/s*xual problems → loss of muscle → reduced strength → increased abdominal fat → poor exercise recovery → lower bone density → mood changes.
However, these symptoms are not specific to testosterone deficiency. Thyroid problems, anaemia, sleep apnoea, depression, nutritional deficiencies and other medical conditions can produce similar symptoms.
How can you naturally support testosterone?
1. Build muscle 💪
Resistance training is one of the most important lifestyle strategies. Prioritise squats, presses, rows, deadlift-type movements and other progressive strength exercises according to your fitness level.
2. Eat adequate protein 🥚🥛🧀🐠🥩🍗
Include eggs, fish, chicken, meat, dairy, whey, paneer or suitable plant proteins. After 50, adequate protein becomes especially important for preventing age-related muscle loss.
3. Don't fear healthy dietary fats 🧈🫒
Testosterone is a steroid hormone made from cholesterol. Include sensible amounts of eggs, fatty fish, nuts, seeds, extra-virgin olive oil and other minimally processed fat sources.
4. Improve metabolic health 🩸🏥
If overweight or insulin resistant, reducing excess visceral fat can help improve the hormonal environment. Focus on whole foods, adequate protein and fibre, and reduce refined carbohydrates and ultra-processed foods.
5. Prioritise sleep 🛌💤😴
Poor sleep and sleep apnoea can negatively affect testosterone. Aim for approximately 7–9 hours of good-quality sleep.
6. Correct nutritional deficiencies 🥦🐟💊
Vitamin D, zinc and magnesium are involved in normal hormonal function. Deficiencies should be identified and corrected rather than taking very high doses unnecessarily.

When should low testosterone be medically investigated?
If testosterone is low and symptoms are present, repeat an early-morning testosterone test and discuss further evaluation with a doctor. This may include free testosterone/SHBG, LH, FSH, prolactin and thyroid assessment.
The goal after 50 is not maximum testosterone. It is healthy testosterone together with good muscle mass, metabolic health, sleep, nutrition and physical function.
