In Oslo, a bronze statue named "Cecilie" stands near the Norwegian Parliament, believed to be the world's first public statue of a woman living with incurable breast cancer. It was modeled on Cecilie Flatval, a mother of three who was diagnosed with metastatic breast cancer at 42.

Most breast cancer campaigns center on survival and beating the disease, which can leave patients with an incurable diagnosis feeling invisible. Around 600 women in Norway die of metastatic breast cancer each year. Cecilie agreed to be the face of the statue to make that group seen, choosing to be shown exactly as she was, including the scar where her breast had been.

The sculpture was commissioned by the Norwegian Breast Cancer Society with the Association of Norwegian Sculptors, created by sculptor Håkon Anton Fagerås, and gifted by property developer Eiendomsspar. Cecilie lived to see it completed before she passed away in April 2023. It now stands permanently in Spikersuppa in central Oslo as a place for families to remember the people they have lost.

ศูนย์สิริกิติ์บรมราชินีนาถ เพื่อโรคมะเร็งเต้านม (QSCBC) ผู้นำด้านการรักษาและการคิดค้นงานวิจัยเพื่อผู้ป่วยมะเร็งเต้านมในระดับสากล โดยเป็นสถาบันทางการแพทย์เพียงแห่งเดียวในภูมิภาคเอเชีย ที่มีศักยภาพและเชี่ยวชาญด้านมะเร็งเต้านมเชิงลึก ได้เข้าร่วมในโครงการวิจัยทางคลินิกครั้งประวัติศาสตร์ระดับโลก "OPTIMA Trial" ซึ่งเป็นนวัตกรรมการรักษาที่ได้รับการนำเสนอในการประชุมวิชาการ ASCO 2026 (American Society of Clinical Oncology) เมื่อต้นเดือนมิถุนายนที่ผ่านมา ประสบความสำเร็จในการใช้เทคโนโลยีตรวจวิเคราะห์ทางพันธุกรรม หรือ จีโนม (Genomic Testing) เพื่อออกแบบการรักษาผู้ป่วยมะเร็งเต้านมเฉพาะบุคคล และช่วยให้ผู้ป่วยมะเร็งเต้านมกลุ่มที่ตรวจวิเคราะห์ทางพันธุกรรมต่ำ (Low Score) ไม่ต้องรับเคมีบำบัด

ความท้าทายของเคมีบำบัด และจุดเปลี่ยนสู่ “การรักษาเฉพาะบุคคล”

โดยทั่วไปภายหลังการผ่าตัดรักษามะเร็งเต้านม แพทย์มักพิจารณาให้ผู้ป่วยได้รับเคมีบำบัดร่วมกับการทำฮอร์โมนบำบัด เพื่อลดโอกาสการกลับมาเป็นซ้ำของโรค ส่งผลให้ผู้ป่วยจำนวนมากต้องเผชิญกับความทุกข์ทรมานจากผลข้างเคียงที่รุนแรงของเคมีบำบัดโดยไม่จำเป็น เช่น อาการผมร่วง คลื่นไส้อาเจียน ความเสียหายของเส้นประสาท และภาวะอ่อนเพลียเรื้อรัง เป็นต้น

โครงการวิจัย OPTIMA Trial จัดทำโดยมหาวิทยาลัยคอลเลจลอนดอน (University College London) และหน่วยงานวิจัยทางคลินิกแห่งมหาวิทยาลัยวอร์ริก (University of Warwick Clinical Trials Unit) ร่วมกับโรงพยาบาลและสถานพยาบาลชั้นนำใน 6 ประเทศ ได้แก่ สหราชอาณาจักร นอร์เวย์ สวีเดน ออสเตรเลีย นิวซีแลนด์ และประเทศไทย ได้พัฒนาขึ้นเพื่อมุ่งเน้นศึกษาในกลุ่มผู้ป่วยมะเร็งเต้านมชนิดตอบสนองต่อฮอร์โมน (Hormone-sensitive breast cancer)

กระบวนการวิจัยของ OPTIMA Trialจะนำตัวอย่างเนื้อเยื่อมะเร็งที่ถูกนำออกมาจากการผ่าตัดไปเข้าสู่กระบวนการตรวจวิเคราะห์ลักษณะทางชีววิทยาและรหัสพันธุกรรมเชิงลึกจำนวน 50 ยีน (PAM 50) เพื่อประเมินความเสี่ยงและทำนายแนวโน้มในการตอบสนองต่อยา จากผู้เข้าร่วมโครงการกว่า 4,000 คน ผลการศึกษาในระยะ 5 ปี ผลลัพธ์ระบุชัดเจนว่า ผู้ป่วยมะเร็งเต้านมกลุ่มที่ตรวจวิเคราะห์ทางพันธุกรรมต่ำ (Low Score) ไม่มีความจำเป็นต้องรับเคมีบำบัดเพิ่มเติม แพทย์จึงสามารถปรับแนวทางการรักษาให้ผู้ป่วยกลุ่มนี้ข้ามขั้นตอนการให้เคมีบำบัด และเข้ารับการรักษาด้วย "ยาฮอร์โมนบำบัดแบบเม็ด" เพียงอย่างเดียวได้อย่างปลอดภัย โดยผลลัพธ์ทางการรักษาและอัตราการไม่กลับมาเป็นซ้ำของโรคเทียบเท่ากับกลุ่มที่ได้รับเคมีบำบัดทุกประการ ช่วยให้ผู้ป่วยได้รับการรักษาอย่างราบรื่นและสามารถกลับไปใช้ชีวิตประจำวันหรือทำงานได้ตามปกติอย่างรวดเร็ว

Asia’s Only Centre in OPTIMA Trial, Optimising Chemotherapy to Improve Patients’ Quality of Life

The Queen Sirikit Centre for Breast Cancer (QSCBC), a leading international institution for breast cancer care and clinical research was selected as the only medical centre in Asia to join the landmark breast cancer research project, the OPTIMA TRIAL. This research programme is a pioneering clinical study whose findings were presented at the prestigious 2026 annual meeting of the American Society of Clinical Oncology (ASCO) in early June.

The research has demonstrated remarkable success in utilising genomic testing to develop personalised treatment strategies for breast cancer patients. This advanced technology, importantly and significantly, enables patients with a low genomic score to safely avoid unnecessary chemotherapy.

The Challenge of Chemotherapy and a Turning Point for “Personalised Care”

Historically, following breast cancer surgery, patients have routinely been treated with a combination of chemotherapy and hormone therapy to reduce the risk of recurrences. This conventional approach, however, has resulted in many patients receiving chemotherapy unnecessarily and experiencing debilitating side effects, including alopecia, nausea, peripheral neuropathy, and chronic fatigue.

Organised by University College London (UCL) and the University of Warwick Clinical Trial Unit, the OPTIMA Trial is a collaborative global study involving leading hospitals and medical institutions across six countries: the United Kingdom, Norway, Sweden, Australia, New Zealand, and Thailand. The research focuses specifically on patients diagnosed with hormone-sensitive breast cancer.

In the OPTIMA Trial, tumour tissue samples collected during surgery undergo comprehensive biological, molecular and genomic profiling across 50 specific genes (PAM50). This advanced analysis enables clinicians to assess the risk of recurrences and predict the likely benefits of treatment, supporting more personalised treatment decisions for over 4,000 participants worldwide.

Five-year follow-up data showed that patients with a low genomic risk score did not have additional benefits from chemotherapy. These findings provide robust evidence that chemotherapy can be safely omitted in this patient group; these patients only require endocrine (hormone) therapy alone.

Clinical outcomes and non-recurrence rates of the non-chemotherapy patients were comparable to those who received chemotherapy. This enables patients to experience a far smoother treatment process, allowing them to recover quickly and with early resumption of their work and daily lives.

As we continue to build the Think Pink Samui initiative, we have been very intentional about ensuring that every event reflects Rotary's motto of Service Above Self. Our project is not a "foreigner's project"—it is a community project for the people of Koh Samui. Wherever possible, we have chosen Thai-owned venues, partnered with Thai businesses, engaged Thai speakers, performers, and volunteers, and welcomed participation from both Thai and international residents. Our International Women's Day event, for example, featured Thai women on our panel alongside international speakers, creating meaningful dialogue that reflected the diversity of our island. By supporting local businesses, fostering collaboration across cultures, and ensuring that our fundraising and awareness activities are inclusive, we are strengthening the entire community. Think Pink Samui is built on the belief that breast cancer does not discriminate by nationality, and neither should our efforts to educate, raise awareness, and improve access to early detection. This is Rotary in action—bringing people together across cultures in the spirit of friendship, service, and lasting impact.
