• Please include full name & number of squares
• Your name will be entered in the grid at random

Payment must be made by February 7th 2025!

HOW TO WIN-
• Numbers will be randomly chosen after grids are full
• Each square represents a score
• Match the last digit of scores at the end of each quarter
