They know when they are losing their memory. That is the part nobody prepares you for.

Families brace for the forgetting. What catches them off guard is the awareness underneath it, the stretch where your parent can feel their own independence slipping and has no way to stop it.

You are losing them. They are losing themselves. Both at once.

I will not pretend that away. But there are so many ways to soften that hardship, in how you talk to them, how the day is structured, what gets decided early instead of in a panic later.

It has become one of my specializations, and lately I am getting calls directly from the person who just got the early diagnosis. Not the family. Them.

Those are some of the most important conversations I have.

Ask what a facility uses instead of a locked memory care wing.

Families assume a place that accepts a wandering resident has a secure unit. Often it does not. And staff cannot sedate someone or restrain them in bed to keep them from getting up.

So what they use is a Wander Guard. A small device on the ankle or wrist that alerts the nursing staff on that floor when a resident leaves their wheelchair or their room.

It is a beeping sound. Sometimes the beep is enough that the resident thinks, I am going to get in trouble, I should sit back down. That is the fall risk prevention.

Whether that is enough depends entirely on your parent. For some families it is. For others it is the reason to hold out for a secured wing with a locked door.

The question to ask on the tour is simple. Do you have a secured memory care wing, or do you use Wander Guards.

There are support groups for memory loss caregivers. There is almost no education.

Those are two different needs and families find out the hard way.

A support group gives you people who understand. That matters enormously and I will always point families toward one.

But when you are in it, mid crisis, trying to figure out whether this behavior is normal for the stage, whether the diagnosis changes the insurance, whether it is time for memory care, you do not need a room of people as lost as you are. You need someone who already knows.

That gap between support and actual education is where I tend to end up, and honestly it is the part of this work I care about most.

All over Chattanooga, families are stuck on the same five questions.

Five questions that decide where your parent lives and what it costs, and most families are answering them for the first time, under pressure, with a hospital waiting on them.

You are not lost because you are bad at this. You are lost because nobody has ever walked you through it.

When the covered days run out, the math starts over.

Families hear a number of covered days in skilled nursing and treat it as the plan. Then the days are up, and the cost has to be reevaluated from scratch.

Here is the part that catches people. A short term bed is often easier and cheaper to find than a long term one, because long term availability at a price your family can actually sustain is the hard thing to secure. So the temporary placement feels fine, right up until it has to become permanent and there is nowhere to go.

The families who come through this well are the ones who started looking at the long term option while the short term one was still covered.

Do not wait for the last covered day to start asking.

Families keep calling me their angel. Here is what I actually do.

I listen. I console. I comfort. And I give people answers they could not get anywhere else, which is usually the part that breaks the dam.

The words that stay with me are the ones like this. I don't know what I would have done without you. And the harder one. I wish I had found you five years ago, before my mother passed.

That second sentence is why I keep talking about this work before families are in crisis instead of after.

One family gifted me a silver necklace with angel wings on it. I do not wear it often, but I know exactly where it is.
