*Reminder- we have received several phone calls concerning brining your own team. If you plan to do so, please fill out the Google form on our page. All players will need to register and pay individually. There is a spot to write your coach’s name, if necessary.

Reminder: If you plan to bring your own team, the head coach will need to sign this form so we can keep track of the number of teams in each age division. Remember we have to have at least 4 teams in that specific age group to play. Otherwise, all players will need to attend evaluations and will be placed on a rec team.
