It was written with the intent of being a reference list for those interested in finding out more. Beware: the only quality assessment covers the longitudinal trials; our confidence in results from other publications isn't much better (some of them are pretty poor, some are deliberately misleading) and I wouldn't recommend making any inference from any single study. If you're interested in finding out more about the BELL trial (the clinical trial component of my PhD) which took place in 2020, here's the protocol on Open Science: https://osf.io/j7uey/

You can also search the hastag on Instagram (and Twitter to a lesser extent), where I've posted a lot (but not all) of the participant feedback. If it's not related to kettlebells (or cake), I'm probably not that excited about it.

How do you communicate your clinical approach to the GPs, surgeons and other healthcare professionals who refer patients to you?

You can tell them that your practice is evidence-informed. You can explain the treatments you offer and the conditions you manage.

But imagine being able to share a published case report demonstrating how you applied research evidence to the management of an actual patient.

Not as proof that a treatment works, or that you're a better clinician than someone else, but as a transparent account of your clinical reasoning, decision-making and patient outcomes.

A case report can provide a meaningful starting point for professional conversations. It gives colleagues an opportunity to understand how you practise, what informs your decisions, and how you navigate the complexities of real-world patient care.

For community-based clinicians, this is a potential benefit of engaging in scholarship that extends well beyond academic publication.

Your clinical work could become something worth sharing with your professional network.

Every day, MSK clinicians make decisions informed by research evidence, clinical experience, and the individual needs and circumstances of their patients.

But how often does that clinical reasoning become visible to anyone beyond the patient and treating clinician?

A case report offers an opportunity to change that.

It allows clinicians to share not just what they did, but why they did it. What evidence informed their decisions? How did they apply that evidence to an individual patient? What challenges or uncertainties did they encounter? And what happened?

Making this reasoning visible creates opportunities for professional discussion, critical reflection and learning across disciplines.

It also gives clinicians something tangible to share with colleagues, professional networks and referring practitioners.

Your clinical experience may have something to contribute to a much wider conversation.

Imagine seeing your name as the first author of a published clinical case report.

Not because you conducted a clinical trial or discovered a new treatment, but because you took the time to critically examine the evidence informing your care, reflect on your clinical decisions, and share what happened with the wider healthcare community.

For many community-based clinicians, publishing their work can seem like something reserved for academics.

It doesn't have to be.

CLEAR supports MSK allied health clinicians to turn their everyday clinical experience into scholarly work, with mentoring, evidence appraisal, peer learning and writing support across an eight-week programme.

The opportunity to become a first author is significant. It's a tangible contribution to the professional literature, something you can include on your CV, share with colleagues and use to open conversations with other healthcare professionals.

And it starts with a single clinical case.

Expressions of interest are now open for Cohort 2.

Most clinicians undertake professional development to improve the care they provide. But how often does that learning result in something tangible that can be shared with colleagues?

CLEAR offers a different approach.

Over eight weeks, community-based musculoskeletal allied health clinicians are supported to critically examine the evidence informing their care, reflect on their clinical reasoning, and develop a case report manuscript based on a patient they have managed.

Along the way, participants have the opportunity to develop their skills in evidence appraisal, academic writing and scholarly communication, supported by mentoring and peer learning.

The aim is not simply to complete another professional development activity, but to produce something that may contribute to the wider clinical conversation.

30 hours of CPD. One clinical case. The opportunity to develop a manuscript worth sharing.

Expressions of interest are open for Cohort 2.

I really don’t like LinkedIn. At all.

The whole professional self-promotion thing is completely at odds with my personality as a task-focused introvert. So, I generally avoid it. Until now. [Sigh]

I’d anticipated that the preprints from our first CLEAR cohort (one of whose authors is kind of a big deal…) might encourage others to step forward and say, ‘Sign me up!’ But that hasn’t happened.

So now I find myself having to spruik CLEAR on LinkedIn, surrounded by academics and others with fancy titles and long lists of post-nominals. And I feel a little bit dirty.

It’s a strange platform. Within hours of posting, I started receiving connection requests from people all over the world I’ve never heard of (many with fancy titles and impressive collections of post-nominals), along with DMs inviting me to submit my work to conferences and journals in India that I’ve never heard of either.

It’s almost as if James and Charles at Wankernomics were onto something…

Fingers crossed my tribe comes through for Cohort 2, so I can quietly retreat from these murky blue waters and get back to doing the actual work.

CLEAR is recruiting for Cohort 2!

Are you a community-based MSK clinician with an interesting case and an evidence-informed approach to patient care?

CLEAR is an eight-week online programme supporting allied health clinicians to turn their clinical reasoning and evidence-informed care into a case report manuscript. No previous academic writing experience required.

Expressions of interest are now open, with Cohort 2 starting soon.

The first Australian case report of the Cross Bracing Protocol was published on 31 May this year—just 43 days before the first CLEAR cohort commenced. Led by diagnostic radiographer Keiley Mead as part of her PhD, it was written with A/Prof Steph Filbay and colleagues, in collaboration with Dr Tom Cross.

It feels fitting that the second Australian case report should come from physiotherapist Geoff Ford at Be Strong Physio in Queensland—a passionate and leading advocate for the Cross Bracing Protocol.

Congratulations, Geoff! I’m incredibly happy to see an idea planted in 2023 finally come to fruition—and excited to see what opportunities it creates.

If you’re an MSK-focused AHP working in the community and interested in osteoporosis exercise, Frances Brown’s new case report is well worth a read.

Frances runs a small-group programme at FKB Physio in Brisbane. The paper follows one patient over 12 months and shows how she used the evidence to guide resistance and impact exercise, while adjusting the programme when low back pain affected what the patient could do.

This is exactly the kind of evidence-informed care happening in community practice that deserves to be shared—and the clinical expertise and reasoning CLEAR was created to help bring into print. Congratulations, Frances! 👏🏼

Twelve months ago, the Clinician-Led Evidence and Reporting project (CLEAR) was little more than a plan on the back of a sandy napkin.

The idea was first pitched at the end of May, from Monkey Mia in Western Australia, and officially became a reality on Monday 30 March this year.

In six short months, we have written the protocol, received ethics approval, developed the CARE-MSK Checklist, created and delivered a considerable volume of programme content, launched a website, and already seen our first brave cohort complete the eight-week programme.

This week, we reached another fairly significant milestone: the first CLEAR-supported case report has been published as a preprint on Zenodo, with another two expected to follow this week.

There is still plenty to learn, refine and improve, but three case reports emerging from our first cohort feels like a pretty good start.

And now we’re recruiting for Cohort 2.

Not bad for something that started on the back of a napkin.

Congratulations to Kendal 👏🏼
