Over the past decade, Ali has created the Trueform Body Mechanics System, the Pain to Peace access programme, and opened the Trueform Pilates Clinic, a purpose-designed space for 1:1 and small group Pilates in Burley-in-Wharfedale, supported by physiotherapists and MSK doctors. “She made me too fit. Now I have to put the shed up.”

That was one of our members (in his 80s), after his morning Pilates session. And we can report that the shed is now up.

We laughed, of course. But this story also gets at something we talk about a lot at Trueform. People come to us with their own histories, concerns and hopes for what they’d like to do. We take time to listen, assess how they move and work with them from there.

For some, the thing that matters is feeling more confident on the stairs, getting down to play with grandchildren or keeping up with the garden.

Of course, we can’t promise that Pilates will get you assembling a shed. In fact, on the evidence so far, it may leave you with fewer excuses.

It’s something we think matters, particularly when people have been dealing with pain, reduced confidence, or changes in their body over time. You can become very aware of what you can’t do, or start moving differently without even realising it.
 
Our memberships give us the opportunity to notice those things. We see how you are moving today, remember how you were moving before, and understand the things that may be influencing it.
 
That might be a change in balance, a movement that has become more protective, or simply something that doesn’t feel quite right. In our sessions it means Ali can respond to the person in front of her rather than asking people to repeat a movement.
 
As another member once put it, ‘Ali knows your body better than you do’.

What happens when you come to Trueform?

It usually starts with a conversation.

You might have been putting up with a problem for a while. Perhaps you’ve had surgery, you’ve stopped doing something you used to enjoy, or you’ve simply noticed that your body isn’t doing what it used to.

So we want to understand what’s going on. What has changed, what you want to get back to, and what matters to you.

From there, your assessment looks at the whole picture. Physiotherapy gives us the clinical assessment, while Ali looks at how your body is moving and where it may be compensating.

That gives us something much more useful than simply knowing where it hurts.

It gives us a starting point.

Your Pilates sessions are then built around what you need, with small groups and people who know your body rather than a generic programme that you just happen to fit into.

And over time, things that once took thought can start to feel like things you simply do again.

There’s a reason strength becomes more important as we get older.
 
Not because we’re trying to turn the clock back, but because everyday life asks quite a lot of our bodies. Getting up from the floor. Carrying shopping. Walking further than usual. Keeping your balance when the ground isn’t quite as forgiving as it looked. Being able to help someone else when they need you.
 
At Trueform, we work with people who want to stay capable for those things. Some come to us after years of being less active because of pain or injury. Others are already active but have noticed that something has changed, their balance, strength, confidence or the way they move.

When your body tightens or pulls back from a movement, it isn’t being difficult. Pain or uncertainty can make the body more protective.

Over time, this can form a feedback loop. If you expect movement to make things worse, you may brace or even avoid it. That gives your body fewer opportunities to experience movement as manageable, so the fear remains, and you may miss out on movement and exercise that could benefit you.

This is where mindset matters. It doesn’t mean telling yourself that pain isn’t real or pushing through regardless. It means becoming curious about your body’s response and beginning to explore what may be possible with the right support.

Research offers some encouragement here. A 2020 systematic review and meta-analysis examined Pilates and Kinesiophobia, the fear of movement associated with pain, in people with chronic non-specific low-back pain. Across five randomised trials, Pilates had a favourable effect on Kinesiophobia when compared with minimal intervention or no treatment. The evidence was rated moderate quality.

The research applies to a particular group, so it cannot tell us what is appropriate for each individual. And that is why assessment matters.

A physiotherapy assessment evaluates your symptoms and the factors that may be contributing to them. A biomechanics assessment then examines your movement patterns and helps identify what you can work on.

In this video, I use a word that may not be familiar: ‘somatic’.

It simply means ‘of the body’. In this context, I’m talking about the way our emotional experiences can also be felt physically, sometimes affecting how we breathe or how safe we feel when we move.
And that doesn’t mean it’s “all in the mind”.
The International Association for the Study of Pain describes it as both a sensory and emotional experience, influenced to varying degrees by biological, psychological and social factors.

People living with persistent pain or lifelong postural difficulties often bring more than physical symptoms to an appointment. Their discomfort may have gradually changed what they feel able to do, as well as how they feel about their body.

That’s why I’m studying for a Master’s degree in Counselling and Psychotherapy alongside my work in physiotherapy and Pilates. It is helping me explore these connections more carefully with clients, while continuing the physical work we already do.

The aim isn’t to suggest that you can simply think yourself out of pain. It’s about understanding your experience more fully and working compassionately with both your body and how you feel.

Movement isn’t only physical. How confident we feel when we move matters too.

It can happen due to all sorts of circumstances, aches make your movements small and protective over time, or a sudden unnerving wobble now makes you worry about falling, there are times when movement feels uncertain, and hesitation can gradually narrow what we do in everyday life.

A 2025 systematic review and meta-analysis, led by researchers at the University of Ottawa and the University of Wolverhampton, brought together 74 studies. It found a small association between greater fear of movement and lower levels of physical activity across several health conditions. The association was also found among older adults, although results varied between studies.

A separate systematic review found moderate-quality evidence that Pilates can reduce fear of movement in people with chronic non-specific low-back pain, compared with minimal or no intervention.

In everyday life, the shift may be quite subtle: fewer “I’d better not” thoughts and more “I could try that.”
Confidence and curiosity can grow gradually as movement begins to feel more manageable, without forcing or rushing it.
