I have evidence in the form of internal emails from the CEO, involving other executives and the local CCG board.

These show Mr Wade amd team breaching their own policies and the NHS Constitution in order to cover poor staffing, poor practice, poor planning, and a lack of understanding of expected basic NHS processes.

Those failures have led to actual harm and even the arrests of innocent service users - that even when Mr Wade is aware and internally admits the error, has not acted to correct.

This has meant vast NHS resources have been spent on defending his personal views and distain for governance , only to concede once the JR process has been started.

I also have further evidence which may help explain the wider failure in compliance, care, governance and leadership.

The current water issue and the response to it should not be viewed as an isolated incident, or as something caused wholly by external factors. It appears to form part of a broader pattern: weak governance, lack of accountability, poor due diligence, and a “make it up as you go” approach to decision-making.

That wider context may also explain why Mr Wade has now departed, apparently to spend “time with his family”.

Mr Wade has basically stated in an email that his personal views are more important than those of the public and what he describes as our ‘perceived right of entitlement to the NHS’

He even claims to powers which are nothing short of ultra vires, bordering on delusional

It’s top down problem by a mediocre mid level manager promoted beyond his skills and should likely never have been given charge of a stationary cupboard let alone a Trust (or two)

As Sir Humphrey would say ‘it’s classic case of someone reaching high office, not through an ability to lead, but a capacity to attend meetings…
