Our bodies are amazing, ladies!!
Just think about what your pelvic floor does when you go through childbirth...
The pelvic floor muscles undergo a huge stretch during a vag.inal delivery.
It is estimated that they stretch to over 3 times their resting length as baby passes through 😳
As well the muscles stretching, the nerves and connective tissue stretch too.
More stretching (and unfortunately, sometimes trauma) can occur if:
- The baby is bigger
- An instrumental delivery takes place (forceps, ventouse)
- If the delivery position of the baby is OP (this is where the baby is born lying on its back and looking up)
- There is a prolonged second stage of labour (2nd stage = pushing phase)

Lessons from a pelvic floor physiotherapist 👩⚕️
Around 1 in 7 deliveries in England involves an episiotomy.
But what exactly IS an episiotomy, and why do they happen?
👉 Episiotomy is a procedure in which an incision is made to widen the va**nal opening during childbirth.
A doctor or midwife will make a small diagonal cut from the back of the va**na, directed down and out to one side. The cut is stitched together afterwards using dissolvable stitches.
- If your baby develops foetal distress (where the baby's heart rate gets faster or slower before birth), an episiotomy may be carried out if a quick delivery is needed.
- It may be necessary to widen your va**na so instruments (such as forceps or ventouse) can be used to help with the birth.
- It may be necessary if you're having a breech birth where the baby is being born with their bottom or feet first.
- It can be helpful if you have been trying to give birth for several hours and are tired after pushing.
👉If you have a serious health condition, it's recommended that delivery should be as quick as possible.
👉Research shows that in some births (particularly with forceps deliveries), an episiotomy may prevent tears that affect the a**l sphincter muscles.
If one of my patients has had an episiotomy, I will assess the area, checking closure of the site as well as formation of scar tissue.
Scar massage can sometimes be helpful if the episiotomy scar is tight and/or painful. The episiotomy area needs to be fully closed before you safely begin massage down there.
