Problem: Severe arthritis + varus + flexion deformity. Femur had subluxed medially off the tibia. Large bone defects in tibia.
Surgery: Staged Complex TKR with stemmed tibial component + screw augmentation to fill defects.
Goal: Get neutral mechanical alignment + stable knee.

*Case 2: Elderly lady with similar varus-flexion deformity + medial subluxation*
Corrected with stemmed TKR + meticulous ligament balancing.

*Why these are “Complex Primary TKR”:*
1. *Not routine surgery:* Standard implants fail. Need stems to bypass weak bone.
2. *Tricks needed:* Stepwise medial release, filling defects with screws/bone graft, checking alignment in extension + flexion.
3. *Planning:* Pre-op CT scans + templating essential. Cannot “wing it” on table.

*Result:* Both patients walking POD-2 with neutral leg alignment.

*Message for patients + referring doctors:*
Don’t wait until deformity is >25° or patient is wheelchair-bound. Earlier surgery = less bone loss = easier recovery.
But even if deformity is severe, correction is possible with proper technique.
