On the occasion of world stroke day, continuing our series on stroke, we wish to create awareness among the caregivers as they play the most important role in the recovery!

When the patient is in the ICU, Physiotherapists help maintain mobility and try for muscle activation. These are such exercises, which if repeated 3-4 times a day by the caregiver, will benefit to get the patient out of the bed faster. Also, many patients have neglect, try to speak to them from the affected side so thay they start recognising the affected side. This helps in getting better muscle activation.

When the patient is out of the ICU, but still in the hospital, we should continue all exercises and caregivers should also learn to make them sit bedside. Therapist and caregiver should make an exercise plan and work accordingly.

After the patients get discharged from the hospital, we should aim at getting the patient to the rehab department for therapy. Getting out of the house, transfers from chair to car etc, travelling all make a lot of difference and help to get the patient’s confidence back! Also, we get to work more vigorously and use various equipments like treadmill, functional electrical stimulation, mirrors, mirror therapy etc.

We want all the care givers to understand that the patient is unable to do any activity because of the stroke! We need to train their brain for it. So be patient and give them time. At the same time don’t pamper them unnecessarily!

Understand that there may be many problems that the patient is facing. So assure them that you are with them for everything they need. But don’t make them learn to be dependent on you!

Many times care givers tell us that the patients don’t do exercises. But it is very difficult for the patients to do all the exercises alone. They need help. Gradually when the strength builds up, the will become independent. Practice is the key to success.

Recovery after stroke

Brain is the Boss!
But one good thing about this Boss is that it can be moulded in any way you or me want. What does this mean?
This is a concept known as neuroplasticity! This means that brain cells can learn things the same way, in same manner, same speed and same intensity they have been taught. Yes that's why we have runners who run differently, or painters drawing the same thing differently.
So now after brain stroke we can shape the new cells of our brain in the way we want. The cells which have died due to stroke will never regenerate again, bit there are abundant cells in our brain which are not doing anything, so we as physiotherapists, take benefit of these cells and teach them the activities and actions of those cells which have died.

Sounds easy but is not at all so... because there are many factors that are stopping this change. Change is not easy we all know, but there's nothing permanent but change!

After stroke one should understand that they have to utilise the weak side of the body to its max, for this they need muscles to work and because the muscles are weak or not working at all the first thing isbto activate them and strengthen them to start moving.
This is the job of a physiotherapist!

Next is as strength starts coming we need to incorporate function and activities in those muscles. This will reshape the brain and new cells of the brain starts doing those lost functions again!

Know your brain!

Your brain is the boss of your body! With the help of billions of neurones and neurotransmitters brain controls memory, emotions, touch, motor skills, vision, breathing, temperature, hunger and every process that regulates our body.

Stroke means brain attack, like heart attack! Brain fights back against the damage caused by stroke and that provides the opportunity for stroke recovery. It depends on the ability of the brain to heal with the help of neuroplasticity. Neuroplasticity occurs when brain cells regenerate, re-establish and re-arrange the neural connections in response to the brain damage.

Physiotherapy, Occupational therapy and speech therapy are designed to spark neuroplasticity. Encouraging the brain to correct physical deficits.
