Clause 9: When Monitoring Becomes Meaningless

Is your data working for you, or are you just working for your data?

We’ve all seen it: dashboards full of Green, Amber, and Red KPIs that get filed away every month without a single conversation.

ISO 9001:2015 Clause 9 (Performance Evaluation) is often where systems go to sleep.

Monitoring is a requirement, but meaningless monitoring is a waste of resources.

Collecting data without analysis and action is like checking your speedometer while driving toward a cliff but never hitting the brakes.

Why actionless KPIs defeat QMS effectiveness:
🎄 False Sense of Security: Seeing a 98% Compliance score feels good, but if you aren't investigating the 2% failure, you are ignoring the root of your next major non-conformity.

🎄Wasted Resources: If the data doesn't drive a decision, the time spent collecting it is a cost with zero ROI.

FSSC 22000 Version 7 Transition: Are you ready for the evolution?

FSSC 22000 has evolved to align with the latest Global Food Safety Initiative (GFSI) requirements.

🔶 PRP Standards Evolution: Version 7 transitions from the ISO/TS 22002-x series to ISO 22002-x:2025. ISO 22002-100:2025 now serves as the common baseline, to be used alongside applicable sector-specific standards.

🔶Strategic Food Defense & Fraud: These are now directly linked to clauses 16.2 and 16.3 of ISO 22002-100:2025. There is a greater emphasis on the competence of the personnel developing and maintaining these plans.

🔶Refined Scope & Categories: Food chain categories and subcategories are significantly more detailed in Version 7, offering a much-improved scope definition for certification.

When Normal Practice Clashes with the System

One important phrase to note in an audit is “This is how we usually do it.”
As an auditor, your ears should be attentive the moment you hear that sentence.

Every company actually has two management systems:
- The Documented System: The clean, perfect procedures stored in the cloud or in binders.
- The Informal System: The shortcuts, tribal knowledge, and workarounds employees use to actually get the job done.

When an auditee says, "The procedure says X, but we usually do Y because it’s faster," they aren't just admitting to a non-conformity.

They are revealing that your documented system has failed to meet the needs of the operation.

How to Audit the Gap:

☑️ Don't stop at the error: If you find someone skipping a step, don't just write a "failure to follow procedure" finding. Ask why the informal process exists.

☑️Look for Shadow Records: Sticky notes on machines or private spreadsheets are some tools people create because the official system is too long or slow.

☑️Audit the Handover: Informal processes thrive at the transitions between departments. Check if the usual way of passing information matches the official communication protocol.

Audit Interviewing Skills; The Power of the Pause

Is your interviewing style helping the auditee hide the truth?

Sometimes, the most effective audit tool isn't a checklist - it’s silence.

Many auditors fall into the trap of over-leading.
They ask a question and, seeing the auditee struggle, they provide the answer for them.

Auditor: "How do you handle rejected material? I assume it goes into that red bin over there and you log it in the ERP, right?"
Auditee: "Yes, exactly."

You haven't audited the process; you've just audited your own assumptions. By leading the witness, you miss the inconsistencies that reveal a real breakdown in control.

3 Pillars of Masterful Audit Interviewing:

➡️Avoid Over-leading auditees: Ask the question and step back. Let the auditee describe the process in their own words. If they struggle, don't fill the gap, let them show you how they truly solve the problem.

➡️Active Listening for Inconsistencies: Listen for the gaps between what is written in the procedure and what the auditee describes. If they say we always do X, but you see Y happening on the floor, that’s your lead.

➡️ Let Silence reveal Gaps: When an auditee finishes an answer, wait three seconds before responding. Often, to fill that uncomfortable silence, people will volunteer the extra detail i.e. the exception to the rule or the shortcut they use when things go wrong.

Audit Questioning Techniques - Why "Yes/No" is a Dead End.

Are your audit questions gathering evidence or just collecting answers?

The difference between a compliance checker and a strategic auditor lies in one skill: Questioning.

If you ask, "Do you follow the procedure?"- the answer will almost always be Yes.
You’ve just confirmed they know the rules, not that they are following them.

This is weak questioning, and it leaves your audit report filled with opinions rather than facts.

To get to the truth, you have to move from "Did you?" to "How do you?" and "Show me."

❌ Weak: "Is your equipment calibrated?"
✔️ Strong: "Can you walk me through the process of verifying this equipment's accuracy before today's shift?"

❌ Weak: "Do you have any customer complaints?"
✔️ Strong: "Can you show me the last three complaints you received and how the team decided on the corrective action?"

When you ask process-based questions, you aren't grilling the auditee; you are exploring the system. You make them provide objective evidence (records, logs, and physical results) rather than just verbal promises.

The Result? You stop looking for mistakes and start discovering how the process actually works in the real world.
