✔️ Photocopy of Birth Certificate (PSA)
✔️ Latest Report Card (SF9)
✔️ Photocopy of any: Latest Billing Statement
✔️ 2 pcs. 2x2 ID Picture (white background)
✔️ Portfolio (if applicable)
