Gabay ukol sa Player Eligibility Approval Form
Para sa lahat ng ating mga players at miyembro, narito ang gabay para sa ating Player Eligibility Approval Form para sa Season 5 (40 & Above Tournament).
Ang mga pisikal na kopya ng form na ito ay ipamamahagi sa court. Tandaan po na ang pagkuha o pagsumite ng form ay hindi pa awtomatikong nangangahulugang pasok na kayo sa draft pool. Ang bawat application ay dadaan muna sa masusing pagsusuri, beripikasyon, at pinal na pag-apruba ng Commissioner, President, at ng Basketball Operations Head.
Mga Bahagi ng Form:
Player Information & Eligibility Checklist: Dito nakasaad ang personal na impormasyon at mga kailangang tuparin tulad ng aktibong pagiging miyembro, paninirahan sa Cavite, magandang asal, at pagsunod sa mga panuntunan at obligasyon sa pananalapi (quota at jersey payments).
Draft Ticket Classification: Nakatukoy dito ang uri ng ticket ng player—Red Ticket (Elite/High-Rated), White Ticket (Regular-Rated), o Blue Ticket (Veteran Player, 60 Taong Gulang pataas).
Draft Pool Status & Committee Remarks: Dito makikita kung ang aplikasyon ay eligible, may kulang pang requirements, sinusuri ng committee, o hindi eligible.
Final Decision & Authorized Signatories: Nangangailangan ng opisyal na lagda at pagpapatibay mula sa Commissioner, President, at Basketball Operations Head bago tuluyang ma-clear ang isang player para sa Season 5 Draft Pool.

To all players and members, please be guided regarding our Player Eligibility Approval Form for Season 5 (40 & Above Tournament).
Physical copies of this form will be provided at the court. Please note that filling out or submitting a form does not automatically guarantee entry into the draft pool. Every submission is subject to strict review, verification, and final approval by the Commissioner, President, and the Basketball Operations Head.

Player Information & Eligibility Checklist: Captures personal details and verifies requirements such as bona fide membership, active participation, Cavite residency, good standing, league conduct, and financial compliance (quota and jersey payments).
Draft Ticket Classification: Categorizes players into Red (Elite/High-Rated), White (Regular-Rated), or Blue (Veteran Player, 60 Years Old & Above) tickets.
Draft Pool Status & Committee Remarks: Tracks whether an application is eligible, pending requirements, under review, or not eligible.
