1. OFFICIAL MILO Little Olympics Registration Form and Waiver*
2. NSO Original Birth Certificate and 2 pcs. Photocopy
3. Form 137/138 with SLN
4. Certificate from the Principal/Registrar that the athlete is a bona fide student of your school (with school seal)
5. Gallery of Athletes (in a separate folder)*
6. Recent I.D. Picture (2x2) – 2 pcs.
7. Certificate of Employment for the Coach/es
8. One (1) empty MILO 1Kg pack per athlete
