In Honor and Remembrance of Cynthia Marie Slater In honor and remembrance of her, we have decided to host the 3rd Annual Cindy Slater Memorial Golf Tournament.

Cynthia "Cindy" Marie Slater passed away on May 20, 2020 - one day before Elevation Chiropractic opened its doors. This tournament is a 3-man scramble and will take place on May 19, 2023 at Swan Lake Golf Course in Clarksville, Tennessee.
