Column 3 of Medical Decision Making isn't about finding the words "prescription drug management" and stopping there.

The 2021 E/M guidelines ask auditors to evaluate the risk of complications, morbidity, or mortality associated with patient management. That means looking beyond the treatment itself and focusing on the documented management decisions and the risk they created for the patient.

If prescription drug management is all that's documented, it may be all you can score. But the goal isn't to check a box—it's to objectively evaluate the documented risk supported by the medical record.

Understanding this distinction is one of the biggest shifts from the 1995 and 1997 guidelines and is essential for accurate, defensible E/M auditing.
