- If one or both birth parents have a different last name than the Minor then legal documentation would be REQUIRED(I.e: a birth certificate).
- LEGAL proof is REQUIRED if there is only 1 parent and/ or guardian (I.e.: adoption paperwork, sole custody paperwork, etc ).
