Requesting a full thyroid panel to get a complete** picture of thyroid function means:
✅ TSH
✅ Free and Total T4
✅ Free and Total T3
✅ Reverse T3
✅TPO and TG Antibodies
AND we also want to look at a CMP and CBC, iron panel in most cases (minimum) with this thyroid panel, as the liver, kidneys, nutrient status and more can impact the thyroid.
Why free and total ?
When hormones are bound to a protein, they can’t bind to thyroid hormone receptors. Only a free hormone can. Measuring total hormones alongside free hormones gives insight into a production vs. binding protein issue.
There are generally 5 places in the hormone chain where things can break down:
Production (the gland level)
Signaling
Transport
Conversion
Cellular uptake
A full thyroid panel, read alongside history, nutrition, lifestyle factors and biofeedback from check-ins, is what lets you actually pinpoint which of those five points needs support. A production issue and a conversion issue may not get addressed the same way.
This is the layer most panels and protocols skip!
But it’s the nuance we teach our mentees to understand along with pattern recognition. And was the topic of this episode ! Because we know you’ve had the case where labs came back “normal” but symptoms were anything but. The full picture may not have been ever looked at.
