Here are some common myths you may hear on the topic:
ACL Myth # 1 ā> āThe ligament that heals after a tear is just SCAR tissueāāØ
This myth has been busted from arthoscopy data from Ihara and Kurosaka all the way back in the 1990ās! The healed structure is FUNCTIONAL ligamentous tissue!
ACL MYTH # 2 ā> āYou canāt surpass the function of a reconstruction for return to sport metrics with non-surgical managment aloneā
This myth has been busted by the KANON trial; the most long-term, highest quality study on the topic ā if healing occurs, function can surpass that of an ACL reconstruction!
However, that *IF* for healing is imperative to accurately assess in your case! We know, via the rule of thirds coined by Frank Noyes, that:
1/3 of tears can heal
1/3 of tears can cope
1/3 of tears need surgery
ā¦so getting clinically evaluated from the team can greatly help ascertain the right route for you!

Common denominators for failure within most rehabiliative programs will include NOT being progressed enough before returning back to sport, or, clinicians giving up on you pursuing rehab once an MRI scan comes back with āfrighteningā terminology.
We can do better than this. The data is out there on ACL tears having the capacity to heal. And even if cases donāt succeed pursuing the non-operative route, there are plenty of high quality coaches and therapists out there who *CAN* get you back to high levels of strength and function AFTER going thru a reconstructive surgery.

ASLAP Squats
As Slow As Possible
No I didnāt use a slow motion camera
I did these on Saturday
Played ~2.5 hours of basketball on Sunday
Legs feel great today; no pain, soreness, etc.
Squatting Low protects you Up High.
And thatās important because every single sport demands you to place your knees āUp Highā in a shallow knee position in some way.
Does your sport require you to run/jump/change direction?
Then you expose your knees in a shallow knee position.
Just like a stronger motor gives you more horsepower, just so a stronger squat gives you more āKnee Powerā; the top end wonāt feel as hard if the bottom becomes stronger.
This is a technique known as improving āJoint Reserveā.

Quick swipe thru post detailing the work of Ihara et al back in 2017 (the year I graduated high school LOL!)
This study had a ton of powerful data on the healing capacity of different ACL tear types.
These are all full thickness ACL tears, but, the fiber orientation was delineated based on if it was straight, curved, displaced, or unclear on initial MRI scan.
āØPooling that data together, healing chances can be delineated based on how each tear type faired with presentation of healing on the second MRI.
The most powerful data I see here is that, Type I and II tear types will lead to ACL re-attachment of some fashion 94% of the time! Leading to function surpassing that of a reconstruction per the data from the KANON trial.āØāØGranted, in this study, the younger the client, the more likely they are to be of those types that arenāt of a high likelihood to heal ā but regardless, the data is powerful in terms of initial MRI presentation:
āØMost tear types at least have a chance!
And, *IF* someone has Type I/II ACL orientation, they have a high prognosis of successā¦. which is why I am seeking to eduacate on this topic.

Indulge the āmemeryā for a moment:
Public perception *is* lacking in this area, and itās up to those in the medical field to change it.
Iām not talking about making exaggerating claims for cases where peopleās knees are literally just mashed potatoes.
Yeah mashed potato knees will likely need cut and drilled into to regain function.
Iām talking about the fully torn ACLs that are clean slices.
We need to educate patients that, just like you can non-operatively manage any other structure of the body successfully, you can also non-operatively manage Grade III ACL Tears to levels of function surpassing that of a reconstruction. Re-attachment can and does occur.
However, so many people are told this is an impossibility.
CAN heal doesnāt mean WILL heal.
But patients deserve to be informed that they CAN.
