Water 💦 Related Injuries & Physiological Changes
Water-related injuries and physiological changes range from mild immersion responses like wrinkled skin and swimmer's ear to severe trauma, barotrauma, and drowning.
Understanding these risks and their triggers is essential for staying safe in aquatic environments.
1. Physiological Changes from ImmersionWhen a human body is submerged in water, the cardiovascular system and skin undergo immediate and distinct changes:
Cutaneous Wrinkling: Skin on the hands and feet wrinkles—a nervous-system response that increases grip in wet conditions.
Blood Redistribution:
Hydrostatic pressure forces fluids from the limbs toward the chest cavity, which slightly increases heart rate and blood pressure.
The Diving Response:
Wetting and cooling the face while holding your breath slows the heart rate and redirects blood to vital organs.Cold Shock: Sudden immersion in cold water triggers an uncontrollable gasp, rapid hyperventilation, and elevated blood pressure, drastically increasing the risk of inhaling water.
2. Acute & Traumatic InjuriesHigh-speed impact or diving into water can cause severe physical trauma:Spinal and Head Injuries:
Diving into shallow water or striking hidden objects can cause severe cervical spine injuries, compression fractures, and traumatic brain injuries.
Water-Force Trauma:
Falling forcefully behind a moving boat or into a personal watercraft jet stream can force water into body orifices, causing internal tissue injury and hemorrhage.
Overuse Injuries:
Frequent swimming can lead to joint issues like shoulder impingement, bursitis, and rotator cuff tendinitis if the swimmer has poor stroke mechanics.
3. Diving and Pressure InjuriesScuba and freediving carry specific risks related to pressure changes and breathing compressed air:
Barotrauma:
As you descend or ascend, pressure differences can damage air-filled spaces in the body, such as the ears, sinuses, and lungs, potentially leading to a ruptured eardrum or breathing difficulties.
Decompression Sickness:
Rapid ascents allow dissolved nitrogen in the blood and tissues to form bubbles, causing joint pain (the "bends") and neurological issues.
4. Drowning and Respiratory IssuesDrowning is defined as the process of experiencing respiratory impairment from submersion or immersion in liquid.
Pathophysiology:
Inhaling water causes the airways to spasm, flooding the lungs and causing oxygen deprivation (hypoxia). This quickly leads to brain damage or cardiac arrest.
Related Conditions:
Rescued victims may still experience lung inflammation, aspiration pneumonia, or Acute Respiratory Distress Syndrome (ARDS).
The outdated terms "dry drowning" and "secondary drowning" are no longer clinically accurate.
5. Infections and Environmental IllnessesNatural water bodies, hot tubs, and poorly maintained pools expose the body to germs:
Ear and Skin Infections:
Bacteria in contaminated water can cause infections like swimmer's ear (otitis externa) or rashes (e.g., "hot tub rash").
Bacterial Infections:
Open wounds exposed to warm brackish or salt water can contract dangerous flesh-eating bacteria like Vibrio vulnificus, which can lead to severe necrotic ulcers and sepsis.
Hypothermia:
The body loses heat much faster in water than in air. Prolonged exposure to cold water drops the core body temperature, causing shivering, loss of coordination, and eventually unconsciousness.
