Disclaimer: This is based on players who usually have not been assessed in pitch rehab and feel they can sprint, run or perform exercises in an isolated environment based of a program their trainer sent them. I have treated many players who have re-injured or delayed recovery because they were just not exposed to footballing scenarios.
Football is not a controlled environment, so return to football should expose the current and future risks to an injury at the right timing.

Many players play at different levels so if you have a shorter break then condense this 10 week program, one thing for certain that should remain is the 2 weeks of rest non-negotiable than the rest you can modify. For most players that 2 weeks has already gone, so time to get to it.
Throughout the off season plan if you have any injuries or niggles get seen to as soon as possible to prevent further delays.
Also this plan is alongside any Football training you do, so work it round that during the week. As we know football fitness is completely different to normal fitness.

If your in the unfortunate position of being injured in off-season, don’t see it as a time to just rest and come back in pre-season without doing the extras to get you back right.
Start now and get yourself right so you can hit the ground running reducing your chance of a reoccurring injury.
