A BIG thank-you to my friends, colleagues, and family around the world for all the birthday love! You sure know how to make a guy from the Bronx feel special—and slightly less concerned about the number of candles on his cake!

Yes, I’ve been a Facebook “ghost” for the past 19+ months.

No witness protection program—just 17 months in Omaha, Nebraska, working, teaching, and learning with the team at Genesis Health & Fitness. What a great experience!

We’re back in Florida now, still kicking butt and raising hell—just with warmer weather and fewer layers!

Thanks again for remembering me, even when Facebook probably thought I’d retired from the internet!

Look-Out-Boston! Here We Come!

Ann and I look forward to discussing emerging best practices that support person-first obesity care programs within the health & fitness sector.

Meaningful progress starts with our support teams understanding these principles, taking the pledge, and putting that commitment into practice.

Together, we can better support people living with obesity.

If you can make it, we’ll see you in Boston!

The Health & Fitness Sector Is Evolving—and We’re Bringing the Conversation to WOC 2026

For the second consecutive year, Ann and I have been invited to share an update with World Obesity Conference attendees, presenters, and the WOC 2026 Scientific Committee on the progress the Health & Fitness Sector is making in supporting individuals at different stages of their obesity-management journeys.

Our message will focus on an important challenge: how the Health & Fitness Sector can become a more informed, collaborative, and valuable part of the comprehensive obesity-management support ecosystem.

At the center of our presentation will be the Person-First Concept—and the fundamental shifts we believe our sector must continue to make:

1. From Weight-Loss Outcomes → To Total Health Outcomes.

2. From Designing People Around Programs → To Designing Programs Around People.

3. From a One-Size-Fits-All Exercise Mindset → To Individualized, Adaptive Programming Options.

4. From “Exercise Harder” → To Selecting the Most Appropriate Physiological & Psychological Stimulus on a particular day & at a particular time.

5. From Prescribe & Expect Compliance → To Listen, Collaborate, Monitor & Adapt.

6. From Pounds Lost → To Health Gained.

This evolution requires much more than creating new exercise programs.

It requires new competencies, better conversations, stronger interdisciplinary partnerships, and a fundamentally different mindset about how we welcome, understand, and support people managing obesity.

The question for the Health & Fitness Sector is no longer simply:

“What program should we offer?”

The more important Person-First question is:

“How should our programming, environment, communication, and support strategies adapt to the person standing in front of us?”

That’s the conversation Ann and I are excited to continue at WOC 2026.

Now we want the Health & Fitness Sector in the conversation.

What are YOU doing differently to help move us from Pounds Lost to Health Gained?

As fitness professionals, we cannot fully understand what an individual managing obesity is experiencing simply by looking at them, weighing them, or completing a fitness assessment.

We must **learn about the person before trying to program the person.**

Every individual brings a different journey.

Some may have experienced unsuccessful weight-management attempts, weight stigma, embarrassment, fear of judgment, low exercise confidence, or frustration with previous programs.

Others may arrive confident and highly motivated.

We cannot assume either experience.

Through active listening, motivational interviewing, empathy, open-ended questions, and collaborative goal setting, we can ask:
What brought you here? What does success look like?

What has or hasn't worked before? What concerns do you have?

What would help you feel supported? How can I best support you?

The answers shift our mindset from "I know what you need" to “Help me understand what you need.”

Please let us know if this infographic helps you.

Thousands of educated fitness professionals around the world have successfully positioned ourselves to provide customer-centered programming and support for individuals at every stage of their obesity-management journey.

We work in wellness centers, leisure centers, health clubs, recreation centers, studios, homes, and other community-based settings.

However, in 2026, the health & fitness sector is still struggling to gain meaningful, positive traction in effectively supporting individuals who are managing obesity.

The attached infographic captures some of the issues that are limiting success within our Sector.

These points are not, by any means, all inclusive. However, they do address issues and challenges that we see globally.

Please post your feedback here so we can continue to learn from each other.

The findings from this study are a wake-up call for health and fitness professionals who are supporting various pathways to success.

The findings reinforce my belief that, regardless of the pathway a person chooses—whether it includes lifestyle change, obesity management medications (GLP-1s), bariatric surgery, or a combination of approaches—our responsibility is not to judge that choice.

Our responsibility is to become a trusted member of their support team from the very beginning of their journey.

By helping individuals build strength, preserve lean muscle, improve function, and create sustainable healthy habits, we have the opportunity to positively influence not only their immediate outcomes but also their long-term health and quality of life.

"See the Person. Honor the Journey. Support the Pathway. Change Lives."

PHILADELPHIA — Most websites selling GLP-1 receptor agonists prescribe medications without any direct interaction with a health care professional, according to data from a “secret shopper” study. Researchers attempted to obtain GLP-1 prescriptions from 49 websites selling the class of medicati...

As a follow-up to this week's posts about the obesity challenges facing Nebraska—and Omaha in particular—I would like to share the attached graphic, which summarizes our guiding principle for supporting individuals on their journeys to improved health and well-being, regardless of where they are in that journey or the pathway they have chosen.

Our philosophy is simple:

**See the Person. Honor the Journey. Support the Pathway. Change Lives.**

This Person-First approach reminds us to look beyond a diagnosis or number on a scale and recognize the individual, respect their unique journey, and provide compassionate, evidence-informed support.

Whether someone is pursuing lifestyle changes, using obesity management medications, considering bariatric surgery, or combining multiple approaches, our role is to meet people where they are and help them move toward healthier, more fulfilling lives.

Our recent post about the obesity spectrum in Omaha, Nebraska generated quite a bit of feedback—both positive and critical—especially around the discussion of bariatric surgery as a treatment option for children and adolescents living with severe obesity.

The responses highlight just how important, complex, and emotionally charged this topic is for many individuals, families, healthcare professionals and fitness professionals.

I still remember the days of playing in the school yards in the Bronx; I remember the days of playing outside with my friends after school until mom yelled out the window “Bobby, Rene…Dinner is Ready”!

I also still remember playing “Hot Peas & Butter” & “Tag” outside on non-school nights.

We can’t forget about “Dodge Ball”, Skateboarding, Roller Skating, Stickball & Handball”!

When I'm out and about in Omaha and see multiple generations of families living with obesity, I find it both personally and professionally moving.

Behind every statistic are individuals, families, and communities navigating complex health challenges.

As a health and fitness professional, it's a reminder that obesity is not simply an individual issue—it is often a multi-generational challenge that requires compassion, support, education, and community-based solutions.

Let’s Continue to support “Journeys-to-Success” with options that are customer-centric.
PS……We’re starting to get traction in Omaha. More to follow.
