The Lifetime Fitness Class ends: Wednesday, May 11th @ 3pm
Here is what you need to complete by then:
1. Finish your 16 / 50 – 60 minute workouts
2. Get a copy of your workout history from Planet Fitness
3. Complete your NWACC Workout Card
4. Take the 40 point quiz on Blackboard
5. Please staple the workout history and any extra credit to your NWACC Workout Card
Gather all of these up and bring them to John Luedtke’s office in Burns Hall room 1432 by Wednesday.
Don’t forget about the extra credit:
· Tanita body composition test for 1 workout
· Blood donation for 2 workouts
Final grades for this class will be posted Monday, May23rd.

Dear Lifetime Fitness Student,
Thanks for registering for the 2nd / 8 week spring semester one credit PEAC #1401 Lifetime Fitness class.
Orientations for all the Planet Fitness Lifetime Fitness as well as the NWACC Fitness Center classes will take place at either the Planet Fitness in
Rogers - 100 N. Dixieland Road Tele. # (479) -636-1111 or Planet Fitness in Fayetteville - 3300 W. Grove Dr. Tele. # (479)-935-4848. The one hour orientation includes: Class policy, fitness assessments and developing a personal work-out program).
Be sure you email me back with one of the below orientation dates that best fits your busy school and work schedule. Students that contact me on or before March 4th and attend the entire one hour orientation will earn an official work-out. *Only 15 Students Max Per Orientation
Note: If you have already attended an orientation or emailed me with your orientation date - please disregard this email!
March 11th (Friday) 3 -4 PM Planet Fitness (Rogers - 100 N. Dixieland Rd) Tele. # (479) 636-1111
March 12th (Saturday) 10 –11 am Planet Fitness (Fayetteville - 3300 W. Grove Dr.) Tele. # (479) 935-4848
March 18th (Friday) 3 -4 PM Planet Fitness (Rogers - 100 N. Dixieland Rd) Tele. # (479) 636-1111
Once you have completed your official orientation, students will then work-out 2 or more times a week for a minimum of 50 -60 minutes per work-out. Students will be graded on your participation as well as how well you document and organize your work-out card.
