If you have had in the past or do currently suffer from any of the conditions listed on the Student Tandem Skydiver Medical Information and Declaration Form (115A) then you must get your doctor to sign & stamp the OTHER form called the –

A LETTER FROM YOUR DOCTOR IS NOT ACCEPTABLE. THE DOCTOR MUST ONLY SIGN & STAMP THE STUDENT TANDEM SKYDIVER REQUEST FOR MEDICAL ADVICE (FORM 115B) IF REQUIRED. THEY MUST NOT WRITE ANY EXTRA INFORMATION ON THIS FORM AS IT WILL INVALIDATE IT. Anyone aged 16 or 17 will require the signature of a parent or legal guardian on the BS Membership Agreement (also known as the under 18 parental consent form). A tandem skydive is the ideal first jump - you will experience the thrill of freefall and enjoy a high performance canopy ride whilst attached to a fully qualified instructor. On arrival you will be introduced to your tandem instructor who will take you through all the training required for you to do your skydive. This includes a talk about the equipment to be used, aircraft orientation as well as covering what you can expect in freefall and finally the position you need to adopt for landing. The training takes about 30 minutes and your instructor will be pleased to answer any questions that you may have. For your jump we will provide you with a jumpsuit, gloves, goggles and headgear. Next you will be fitted into the dual harness. You will be jumping from up to 12,000 feet. It takes around 15 minutes for the aircraft to reach altitude so you will have time to relax and enjoy the spectacular views. Once at altitude, you are ready to go! You will be freefalling at 120mph for about 45 seconds. Your instructor will deploy the parachute by 5,000 feet. The parachute has dual controls so you can aid the instructor by spiralling the parachute around the sky. Your instructor will take control of the parachute for the landing. All in all it is an experience guaranteed to take your breath away! To commemorate your skydive you will be presented with a certificate at the end of the jump. The cost for the tandem skydive, including the instructor payment and third party insurance, is £240 per person. A deposit of £50 is required at the time of booking. Our primary aim at Skydive Swansea is to promote and help Welsh and national charities raise funds through tandem skydiving by offering a direct service. By dealing with the parachute centre direct, additional fees will not be unsuspectingly paid to agencies and middlemen for making a booking on your behalf. Our pledge is that every penny above the cost of the tandem skydive goes to your chosen charity. Through your efforts at fundraising you will have the option to experience the thrill of skydiving and help raise much needed funds for charity." You are required to raise a minimum of £395 in sponsorship. The cost of your skydive is paid out of your sponsorship money and the rest goes to the charity that you have chosen. So by raising the minimum of £395, £230 pays for your skydive and £165 goes to the charity. The payment for your jump includes the instructor payment and third party insurance. If you raise more than the minimum amount, the costs of the skydive remain the same, and the charity gets more money. We will provide you with sponsorship forms when we have received your deposit. If you wish to pay for the skydive yourself then we ask that you raise a minimum of £165 for the charity of your choice. Please note that on the day of your jump you are required to pay for the balance of your skydive with cash or postal order. If you have used Just Giving to raise all of your sponsorship online then you will not be able to do this. Therefore we suggest that some sponsorship money is raised using the sponsor forms provided in order to be able to jump for free. All that is required to book your skydive is a deposit of £50. This can be paid by cheque or postal order made payable to "Skydive Swansea Ltd". Please note that the cost of a camera option cannot be paid for from your sponsorship money. Ground Rush - an adrenaline rush from 7,000 feet! Experience a different type of tandem where you exit from a lower altitude of 7,000 feet instead of our usual 12,000 feet. A popular choice for on a budget for those wishing to try before embarking on the higher tandem. We can also take men up to 16 stone on this tandem. Training is to the same high standard as our regular tandem skydives. On arrival at the parachute centre your tandem instructor will take you through all you need know to make the jump safely. This will include the equipment to be used, aircraft orientation as well as covering what you can expect in freefall and finally the position you need to adopt for landing. This briefing and training generally takes 30 minutes and the instructor will be pleased to answer any questions you may have. For your skydive you will be provided with a jump suit, gloves, goggles and headgear. Next you will be fitted into your harness by your instructor. Once inside the aircraft you will be attached to your instructor. You will still experience that huge rush of adrenaline as you see the door open at 7,000 feet and you move towards the edge of the aircraft. Momentarily suspended outside before you exit. Your instructor will deploy the parachute within seconds and by about 5,000 feet you'll be under a Ram Air Tandem Parachute spiraling through the sky. The parachute has dual controls and you can aid the instructor in steering the parachute to make the most of the stunning views over the Gower peninsula. Your instructor will take complete control of the landing after about 5 minutes, guaranteed to take your breath away! To commemorate your skydive experience you'll also be presented with a certificate of achievement. We will be offering the Ground Rush Tandems on Fridays and Bank Holidays throughout the year.
